Jake Reyna-Casanova is currently the Grants and Patient Advocacy Manager at CommuniCare in San Antonio, TX. He has worked in federally qualified health centers (FQHC) for over 8 years focusing on development, grant writing, and strategic planning for these crucial organizations in our communities. Currently, CommuniCare is an FQHC providing healthcare services to increase health equity in and around the San Antonio and South Central Texas regions. His primary responsibility is to research, write, and manage all the grants for the organization. He has also recently implemented a new Medicare program and now oversees a team who assists patients in the education, knowledge, and enrollment into Medicare programs. He is passionate about health equity and encourages his team and leaders at CommuniCare to continue being innovative to enhance services offered to the diverse population in the community he lives in. He provides assistance to the Chief Executive Officer working on special projects, as assigned.
Jake received his Bachelor’s in Public Health, with a concentration in nutrition, from The University of Texas at Austin in May 2014. He received his Master’s degree in Public Health concentrating in Healthcare Management in May 2016.
Jake and his husband enjoy finding new places to eat in San Antonio, being outdoors & active, and spoiling their four fur babies!
